2024 CPSD Swimmer Registration Form

Please know that, in accordance with Tar Heel Swim Association (TSA) rules, Swimmer Registration requires being a Cary Park Swim Club member in good standing.

This means that you MUST have applied for, and paid for a Cary Park Swim Club Family Membership (see carypark.org for more information), which will then be confirmed by the POA management company before we can fully activate your paid-for Cary Park Sea Dragons Swimmer Registration(s). You can complete the swim team registration process before receiving your swim club membership, but your swimmer(s) will not be allowed to practice until we have confirmation of your swim club membership from Elite Management.

Again, before your kids can practice, all registrations for Cary Park Sea Dragons first REQUIRE:

  • Active Cary Park Swim Club Family Membership - Applied, Paid, and Confirmed

Note that non-Cary Park residents must mail their payments to Elite Management, so to take advantage of the Early-bird discounts for pool membership, please remember to leave time for your check to arrive to Elite Management by March 15th!

Family Volunteer Service Requirement

Required Parent Participation Policy - Family Volunteer Service Requirement

Upon swimmer(s) registration for Cary Park Sea Dragons, your family is under obligation to complete 12 Volunteer Service points over the course of the summer swim season. These points are monitored to assure that our swim team community can run a successful swim season for our children. These points do not carry over into following years. Half (6) of the points must be signed up for by June 30, 2024. Failure to fulfill this vital obligation will result in the removal of the entire family's swimmer(s) from the Cary Park Sea Dragons (ability to practice or participate in swim meets) and jeopardize the following year's eligibility. No refunds will be given in these situations.

The ONLY Exemptions from Required Volunteer Service Points are if you have ONLY a Mini Sea Dragon or ONLY a 15+ swimmer.

The Cary Park Sea Dragons Board of Directors reserves the right to suspend or dismiss any Family/Swimmer(s)' from the Cary Park Sea Dragons Swim Team for any reason, including but not limited to, failure to meet your required 12 Volunteer Service Points, Violation(s) of the Sportsmanship and Good Behavior Code of Conduct, or other actions that impede or inconvenience the ability of the Sea Dragons Swim Team to function properly. Suspensions or Dismissals will be executed without Refunds.

Liability Waiver

I, my heirs, executors, administrators and assigns, hereby release the Cary Park Sea Dragons, Cary Park Swim Club, Pool managers and operators, volunteers, coaches, and board of directors, Cary Park Homeowners Association, and Tarheel Swimming Association, from injury or loss to the above named participants (swimmers), and also any family members, which might occur during participation and travel to and from planned events, including injury or loss arising from or contributed to by any physical impairment or defect that said participants may have, whether latent or patent, and I agree that sponsors and promoters are under no obligation to provide physical examination or other evidence of said participants' or family members' fitness to participate in such activities, the same being my sole responsibility.

I represent that myself, my spouse, my swimmer(s), and my family are in good health and proper physical condition to participate in any and all the activities which Cary Park Sea Dragons, Cary Park Swim Club, and Cary Park HOA provides. Further, I represent that myself, my spouse, my swimmer(s), and my family have not been advised against these activities by a health professional.

I represent that my swimmer(s) and my family have insurance through my own insurance carrier, and that only the vehicle owner's liability coverage is applicable to any vehicular accident en route to any swim team activities.

I am specifically consenting my swimmer(s) and my family to participate in Cary Park Sea Dragons activities, recognizing the risks inherent in such activities.

In the event of any accident or medical emergency, I authorize Cary Park Sea Dragons coaches or board of directors or Cary Park Swim Club lifeguards to seek and consent to emergency medical assistance on behalf of my swimmer(s) and family. I will assume responsibility for all expenses. Furthermore, I understand, Cary Park Sea Dragons will use any provided contact information to contact me in the event of an accident or medical emergency.

RELEASE OF LIABILITY:

Despite all known and unknown risks including but not limited to serious bodily injury, permanent disability, paralysis and loss of life, I, on behalf of myself, my spouse, minor child(ren)/ward(s), and family, hereby expressly and voluntarily remise, release, acquit, satisfy and forever discharge and agree not to sue Cary Park Sea Dragons, Cary Park Swim Club, and Cary Park HOA, including suppliers, designers, installers, manufacturers of any pool equipment or materials, and facilities, and agree to hold said parties harmless of and from any and all manner of actions or omission(s), causes of action, suits, sums of money, controversies, damages, judgments, executions, claims and demands whatsoever, in law or in equity, including, but not limited to, any and all claims which alleged negligent acts and/or omissions committed by Cary Park Sea Dragons, Cary Park Swim Club, and Cary Park HOA, equipment suppliers, volunteers, coaches, and board of directors, whether the action arises out of any damage, loss, personal injury, or death to me or my spouse, minor child(ren)/ward(s) and family while participating in or as a result of participating in any of the activities in or about the premises.

This Release of Liability is effective and valid regardless of whether the damage, loss or death is a result of any act or omission on the part of Cary Park Sea Dragons, Cary Park Swim Club, Cary Park HOA, equipment suppliers, volunteers, coaches, or board of directors.

In the event of any filings against Cary Park Sea Dragons, Cary Park Swim Club, and Cary Park HOA, I agree to the sole and exclusive venue of Cary Park Swim Club, 5353 Cary Glen Boulevard, Cary, NC 27519.

I further agree that the substantive law of North Carolina shall apply without regard to any conflict of law rules. By signing this document, I understand that I may be found by a court of law to have forever waived my and my spouse, child(ren)/ward(s), and famiy right to maintain any action against Cary Park Sea Dragons, Cary Park Swim Club, Cary Park HOA, volunteers, coaches, board of directors, on the basis of any claim from which I have released Cary Park Sea Dragons, Cary Park Swim Club, and Cary Park HOA, and any released party herein and that I have assumed all risk of damage, loss, personal injury, or death to myself, my spouse, and/or my minor child(ren)/ward(s), and family and agreed to indemnify and hold harmless Cary Park Sea Dragons, Cary Park Swim Club, Cary Park HOA, equipment suppliers, volunteers, coaches, and board of directors from and against any and all losses, liabilities, claims, obligations, costs, damages, and/or expenses whatsoever paid, incurred, and or suffered by Cary Park Sea Dragons, Cary Park Swim Club, Cary Park HOA, equipment suppliers, volunteers, coaches, and board of directors as a result of the participation in activities in or about the facility by myself, my spouse and/or child(ren)/ward(s), and family and/or claims asserted by myself, my spouse and/or child(ren)/ward(s), and family against Cary Park Sea Dragons, Cary Park Swim Club, Cary Park HOA, equipment suppliers, volunteers, coaches, and board of directors related to such participation in said activities.

Medical Release Waiver

I certify that I am the parent or legal guardian for my child(ren). I hereby give my permission for any supervisor, coach or other team administrator associated with the Cary Park Sea Dragons to seek and give appropriate medical attention for our child(ren) in the event of accident, injury, illness. I will be responsible for any and all costs associated with any necessary medical attention and/or treatment.

I hereby waive, release and forever discharge Cary Park Sea Dragons and associated supervisor, coach or other team administrator from all rights and claims for damages, injury, loss to person or property which may be sustained or occur during participation in Cary Park Sea Dragons activities, whether or not damages or loss is due to negligence. I hereby acknowledge that my children is (are) physically fit and capable of participation in all Swim Team activities.

Photo Waiver

Cary Park Sea Dragons Swim Team reserves the irrevocable right and permission to photograph or video or record above named participants and family members during activities in connection with Cary Park Sea Dragons, Cary Park Swim Club, and Cary Park HOA, and to utilize photographs or video or recordings through any media or in any manner it deems proper, in perpetuity throughout the world and without restriction as to alteration, and I waive any right to inspect or approve said photos/videos/recording of myself, my spouse, and/or child(ren)/ward(s), and I further agree that the rights granted to this release include my waiver to any right to any compensation whatsoever from said photos/video/recording.

Code of Conduct Policy

CARY PARK SEA DRAGON SWIM TEAM SPORTSMANSHIP AND GOOD BEHAVIOR

We expect Excellent Sportsmanship and Excellent Behavior at all times. This applies to both Adults and Children, Swimmers and Fans alike, and to our Own Teammates as well as to Our Competitors. This is very important for the Spirit of Healthy Competition, for our Children's Safety, and for the Enjoyment of All. Plus, it reflects on who we are individually and as a team.

Sportsmanship - fair play, respect for opponents, and polite behavior by someone who is competing in a sport or other competition.

Behavior - 1) the manner of conducting oneself; anything an organism does involving action and response to stimulation; the response of an individual, group, or species to the environment; 2) the way in which someone behaves; 3) the way in which something functions or operates.

Particular actions by Parents, Families, or Swimmers that will absolutely not be tolerated include, but are not limited to:

  • Abuse of or damage to facilities or equipment
  • Abuse of or damage to other swimmers' belongings
  • Swearing, verbal or physical assaults
  • Disrespectful behavior towards Coaches, Swimmers, Officials, or Anyone affiliated with TSA, or Swim Fans
  • Presence of alcohol at a Swim Meet

Parents, Families, Swimmers:

Respect all other athletes, coaches, fans, and pool staff. Promote positive team spirit and morale.

VIOLATIONS OF EXCELLENT SPORTSMANSHIP AND EXCELLENT BEHAVIOR

could result in dismissal from practice or swim meets, parental notification, suspension or dismissal from the Sea Dragons Swim Team as determined by the Cary Park Sea Dragon Swim Team Coaches and/or Board of Directors.

The Cary Park Sea Dragons Board of Directors reserves the right to suspend or dismiss any Family/Swimmer(s)' from the Cary Park Sea Dragons Swim Team for any reason, including but not limited to, failure to meet your obligated 12-point Volunteer Service Requirement, Violation(s) of the Sportsmanship and Good Behavior Code of Conduct, or other actions that impede or inconvenience the ability for the Sea Dragons Swim Team to function properly. Suspensions or Dismissals will be executed without Refunds.

We understand this policy, and our family (Parents, Swimmers, Family) agrees to abide by the Cary Park Sea Dragons Code of Conduct.

Refund Policy

Any child that is not able to proficiently swim 25 yards unassisted multiple times, without stopping, as determined by the coaching staff by the end of the second week of practice will be asked to participate on the pre-competitive team (if they are 8 or under and able to swim at least 10 yards) or to try again next year.

Before the swim season has started (June 1), a full refund of the Cary Park Sea Dragons Swim Team Registration will be given.

No refunds will be given after the initial trial period (June 1).

No Refunds will be given for suspension or dismissal from the team or for forfeiture of swimmer(s)' awards if the 12-Point Volunteer Requirement is NOT met.
